u1.net will pay for the cost of the individual employee coverage for medical insurance through AmeriHealth. Family members of the employee may be provided coverage at an additional cost. As an added benefit, employees will be eligible for enrollment in the AmeriHealth benefits plan immediately. Coverage becomes effective on the 15th of the month following date of hire (The 1st of the month as of April 1999). Upon becoming enrolled, you will receive a Member ID Card and a Provider Directory. The directory lists over 140 hospitals and thousands of physicians and specialists. In-network costs are: $10 co-pay for doctor visits and $5 co-pay for generic drugs; $10 co-pay for brand-name drugs. In recognizing the financial burden of the PPO plan for a family, u1 also provides an HMO through AmeriHealth. Dental insurance is offered through GE FInancial Assurance. Additional coverage may be obtained for other family members through a payroll deduction. Coverage begins on the 1st of the month following a service period of 30 days. u1.net's dental insurance covers 100% of preventive service, 80% of basic services, 50% of major services and 50% of orthodontic services (Orthodontic benefits are available to all eligible dependent children under age 19). Please refer to the Dental Summary of services included in the Welcome Pack for more details regarding deductibles and features of this plan. |

u1.net covers all active, full time employees from their date of hire. This life insurance policy covers 1x salary, where salary is considered base earnings only to a maximum of $200,000.00. This is the same amount for Accidental Death & Dismemberment. This benefit is reduced based on the age of the employee according to the following schedule:

Short-Term Disability u1.net covers all active, full time employees from their date of hire. The short-term disability benefit supplements the State of New Jersey's short-term disability coverage providing 67% of weekly earnings to a weekly maximum of $1,500. Short-term disability coverage has a maximum duration of 6 months, after which time the Long-Term Disability coverage takes over. Long-Term Disability u1.net covers all active, full time employees from their date of hire. The long term disability policy come into effect 6 months after a disability is established, which is immediately after the coverage of the Short-Term Disability policy stops. Beginning June 1, 2000 all u1.net employees and their family members may take advantage of a vision plan program called EyeMed Vision Care. EyeMed Vision Care Plan is a service of BeneServ and is provided at no cost to u1.net employees and their families. The EyeMed Program offers immediate savings on eye care needs including both eyeglasses and contact lenses. Savings will be up to 50% on frames, up to 20% on lenses and about 15% on contacts. You can also get 20% discounts on items such as contact lense solution and non-prescription sunglasses. The EyeMed Program has independent optometrists, opticians and ophthalmologists, which include LensCrafters. They have 3300 provider locations with over 7700 providers. EyeMed utilizes a wide variety of frames from the world's largest frame manufacturer, Luxottica. Luxottica manufactures frames for the leading brands in the optical industry including: Giorgio Armani, Ann Klein, Yves St. Laurent, Brooks Brothers and many others. u1.net's holiday and paid time off policy is designed to provide to the permanent full-time and part-time employees the most flexibility in managing their time off. u1.net does not provide any paid time off to hourly employees. u1.net provides permanent employees with only 2 classes of paid time off, six holidays that are fixed and an 'allotment bank' of time based on years of service. Holiday Paid Time Off u1.net has elected to recognize the following holidays:
The remaining 'standard/typical' holidays have been incorporated into the 'allotment bank' to give the employee greater flexibility in taking paid time off. If the client requires you to work on any of the above holidays, u1.net will pay you for both the holiday as well as the actual hours worked. The holiday time is not flexible and can not be taken on another day. Each employee should familiarize themselves with the clients' holiday schedule. If the client has more than six (6) holidays within a given year, the consultant will need to use his/her other allotment of time off to coincide with their clients' schedule. Allotment Bank u1.net provides its employees with annual paid time off, this covers vacation, and personal and sick time off. Under the policy, all employees receive an allotment of time-off (based on a percent of full-time hours and u1.net tenure). This 'bank' of time is anniversary year based and so it commences with an employee's start date and ends with the employee's last day of employment with u1.net. The manner by which this allotment bank is accrued for each full-time u1.net employee is based upon the length of service as defined below.

Bereavement Policy
u1.net recognizes that certain family emergencies occur and that they provide relief to the employee during such occurrences, as described below:

In 1998 u1.net developed a bonus plan to promote its core values of responsibility, honesty, and commitment. The plan is based upon the company reaching its objectives. Scope & Objectives The plan is designed to help the company promote a team-oriented and long-lasting atmosphere. And to say, "thank you" for contributing to the success of u1.net. Participation & Eligibility The plan will apply to all employees as of 17 April 98. Accruals are not permitted in this plan, and an employee must be employed by u1.net, as an employee in good standing, at the time of pay out. Basic Plan The plan is based upon the semi-annual performance of the company. Presently, the company will use Operating Income (OI) as a gauge to determine the parameters of the plan. Others may be added at any time. Currently, the scale is used to show company performance in terms of OI and the corresponding factor. This factor, if any, is applied to gross wages earned during the 6 month period. For example: if gross wages were $1,000.00 for the period, and the company performance results in a factor of 2.75%, then this person would receive $27.50. As part of the planning process, each January and July the company will review the prior six months of company performance. During this review, the next six months and the following year business plans will be developed. These plans will be evaluated and agreed upon by the Executive Committee. Pay Out
The pay out of the plan will be semi-annually: 1)
during the accounting period that follows the end of the
first half of the year, and 2) during the accounting period
that follows the year end. A pay out (and subsequent
ones if necessary) will reconcile any balances that an
employee may have at that time.

u1.net realizes the importance of both personal and professional development, in all forms, for its employees. In this guide "enhancement" or "development" will refer to any means of improvement to an employee's current professional level. Improvement is limited only by each employee's imagination. Enhancement may include, but is not limited to: college tuition, seminars, conferences, classes, software, self-paced interactive curricula, etc. This policy is u1.net's way of setting the guidelines to empower its employees with a means to heighten their professional abilities. u1.net views training in any form as a way to enhance the employee as well as the company. It is very important that the enhancement be passed along to others in the company. This can take the form of sharing materials, preparing formal training, and (minimally) a UP report, which will be posted on u1.net's website. Basic Enrichment u1.net will allow each employee $3,000.00 per year to pursue any professional improvement as defined above. The request for basic enrichment must be accompanied by a form verifying client or immediate manager approval for time off as well as a description of the intended enrichment path. The $3,000.00 will include all travel and expenses for the approved enhancement. If the basic allowance is not used within a year, it will roll over and be added to the following year. The employee can not accumulate more than $6,000.00 basic enrichment dollars. The allowance must be used as approved enrichment. Unused funds will roll-back into the u1.net enrichment fund. Extended Enrichment If a u1.net employee requires training or enhancement in addition to the basic enrichment a more formal procedure of approval will be followed. In addition to the basic enrichment approval also a written request will be submitted to a peer review. To qualify for extended enrichment the employee must clearly define the path, goal and knowledge transfer. Just to highlight 2 of the many issues associated with local travel ... Personal Vehicle Use
Any business travel involving use of your personal vehicle is reimbursed
at the government approved rate of $.32.5/mile (approved for use on
Jan 14, 2000). The rate has been calculated by the GSA to cover all
normal vehicle expenses: gas, oil, maintenance, and insurance for the
average vehicle.
